haha, yea it isn't a very comforting fact, is it. As long as they are specially licenced for it then you should be good :) As far as universal definition, unfortunatly there is none. It all depends on your insurance and what plan you have. I was doing years of researching about it before I got mine (because I had to wait till I was done growing of course) but I think when it got down to the time to do it, it probably took me about 6 months.
I was originally a 36E and right now I am down to a 36D. I still have a bit of swelling, so I'm hoping they will decrease to a C (which is what I wanted), but they might not and I would still be ok with that. Unfortunatly they doctor took out so much tissue (over 1000 grams) that he was afraid to take out anymore and cause too much stress to my body.
I have before and after photos, however the after photo was taken directly after the procedure so I was still not allowed to wear bras and thus they look a bit smaller then they really look now (because they were smushed down with Ace bandages and I wore a medical bra thing to provide with some sort of support for me.) but you still get the jist.
here are photos:

afterwell that was my experience in a nutshell.. I hope that I helped you out a bit and made you feel more comfortable with the surgery. I really has been one of the best choices I've ever made!
feel free to always come to me with any questions or concerns you have regarding the procedure and I will gladly answer to the best of my ability! And good luck with whatever you decide to do! I hope everything will work out for you :)
